    Women with mutations in the BRCA1 or BRCA2 genes can significantly reduce their risk of certain types of cancer by having their ovaries removed, according to a study in the July 12 issue of JAMA. Women with a harmful mutation in the BRCA1 or BRCA2 gene have a high lifetime risk of ovarian cancer (range, 15 percent-54 percent), and mutations in either of these genes increase susceptibility to cancers of the ovary, fallopian tube, and peritoneum (abdominal lining), according to background information in the article. Women with this mutation are often advised to undergo preventive oophorectomy (surgical removal of the ovaries). The effectiveness of this intervention has not been prospectively evaluated in a large group of patients. Steven A. Narod, M.D., of the Toronto-Sunnybrook Regional Cancer Center, Toronto, Ontario, and colleagues conducted a study to determine the absolute risks for developing ovarian, fallopian tube, and peritoneal cancers in a group of BRCA1 and BRCA2 mutation carriers and estimated the risk reduction associated with salpingo-oophorectomy (surgical removal of the ovaries and fallopian tubes). Women known to carry a BRCA1 or BRCA2 mutation were identified from an international registry between 1992 and 2003. A total of 1,828 carriers at 1 of 32 centers in Canada,discount trx suspension trainer, the United States, Europe, and Israel completed questionnaires at baseline and follow-up. Participants were observed from the date of study entry until: diagnosis of ovarian, fallopian tube, or peritoneal cancer; death; or the date of the most recent follow-up. After an average follow-up of 3.5 years, 50 new ovarian, fallopian tube, and peritoneal cancer cases were reported in the group. Of the 1,828 women, 555 (30 percent) underwent preventive removal of the ovaries and tubes prior to study entry, 490 (27 percent) underwent the procedure after entering the study, and 783 (43 percent) did not undergo the procedure. There were 32 new cancers diagnosed in women with intact ovaries,trx training bands. The estimated cumulative incidence of peritoneal cancer is 4.3 percent at 20 years after oophorectomy. The overall (adjusted) reduction in cancer risk associated with bilateral oophorectomy is 80 percent. The researchers estimated the risk of ovarian cancer to be 62 percent for BRCA1 carriers and 18 percent for BRCA2 carriers in women up to age 75 with both ovaries intact. "Women who carry a mutation in the BRCA1 gene are asked to consider prophylactic bilateral salpingo-oophorectomy at age 35 or thereabouts,weider x factor home gym, in order to reduce the risk of ovarian, fallopian tube, and breast cancer. Our observations support this recommendation. It may be reasonable to wait until a time closer to menopause to prevent ovarian and fallopian tube cancer in BRCA2 carriers but this delay will diminish the level of protection offered against breast cancer in this subgroup," the authors write. "We estimate the magnitude of the risk reduction [from preventive removal of the ovaries and tubes] to be approximately 80 percent and the residual risk of 4 percent of peritoneal cancer is not sufficiently high to recommend against the procedure. It is important that both the fallopian tubes and ovaries be removed because either site may be the origin of cancer and both organs should be examined in fine detail to rule out the presence of microscopic disease," the researchers conclude.     Falls More Likely In Elderly Men With Low Testosterone Level

    Low testosterone levels may be associated with a higher risk of falling in older men, according to a report in the October 23 issue of Archives of Internal Medicine, one of the JAMA/Archives journals. Some researchers have speculated that this decline in sex hormone levels contributes to health conditions associated with aging; however, few studies support this notion. Muscle weakness and muscle loss are two adverse events that often occur with age and are known to increase the risk of falling among older adults. Eric Orwoll, M.D,trx shop., Oregon Health & Science University, Portland,trx force training kit, and colleagues studied 2,587 men age 65 to 99 (average age 73) who enrolled in the Osteoporotic Fractures in Men (MrOS) Study between 2000 and 2002. At the beginning of the study, blood samples were taken and participants filled out questionnaires about their medical history, medications and lifestyle habits. The men completed several physical performance tests, including those assessing grip strength, leg power and balance. Every four months, they reported whether they had fallen and if so, how many times. During the course of the study, which continued until March 2005, 56 percent of the men fell at least once. Those with lower bioavailable testosterone levels were significantly more likely to fall and to fall multiple times than those with higher levels--among the one-fourth of participants with the lowest testosterone levels, the risk for falling was 40 percent higher than among the one-fourth with the highest testosterone levels. The association was stronger in younger men (ages 65 to 69) and not apparent in men older than 80. The association remained the same when the researchers factored in the scores on physical performance tests. "Bioavailable testosterone concentration is associated with measures of physical performance, but the association of testosterone level to the risk of falling is apparent regardless of physical performance," the authors write. "Thus, the mechanisms by which testosterone level affects the propensity to fall may involve other pathways." For example, low testosterone levels could impair vision, thinking processes or coordination, increasing the risk for falls. "These findings strengthen the link between testosterone and the health of older men, suggesting that the effects of testosterone on fall risk may be via novel mechanisms and provide insight into how testosterone measurements might be useful for identifying men at higher risk for adverse events," the authors conclude. Pregnancy & Childbirth "Neonatal Mortality and Morbidity Rates in Term Twins With Advancing Gestational Age," American Journal of Obstetrics and Gynecology: Jennifer Soucie and colleagues at the University of Ottawa's OMNI Research Group examined the records of 60,443 twins born between 1995 and 1997 in the U.S. after at least 37 weeks' gestation (BBC News, 8/11). The study finds that infants born after 40 weeks' gestation are more than 2.5 times as likely to die as twins born at 37 weeks' gestation. In addition, twins born after 40 weeks' gestation have a 74% higher chance of having low Apgar scores -- which measures heart rate, breathing, muscle tone, color and response to stimulus -- as twins born at 37 weeks' gestation. There is no significant change in the mortality rates of infants born at 38 or 39 weeks' gestation, compared with those born at 37 weeks' gestation. In addition, infants born during week 39 of gestation are 17% less likely than infants born at 37 weeks to require breathing assistance. Preterm birth, Proceedings of the National Academy of Sciences: Jerome Strauss, dean of Virginia Commonwealth University's School of Medicine, and colleagues discovered a variant in a gene that directs the production of collagen and is a found in the amniotic sac that lines the uterus of a pregnant woman. Researchers reported a "statistically significan[t] increase" in the total number of breast calcifications in the group with decreased kidney function, compared with the control group (American Journal of Kidney Diseases, 8/21). Among women in the dialysis group, 84.4% developed breast calcifications, compared with 59.0% of women in the control group, the study finds. The researchers concluded, "Every woman on dialysis therapy should be referred for screening mammography, and the physician taking care of these patients should be aware of the slightly increased risk for workup and possibly resultant psychological stress after screening dialysis patients" (Reuters Health, 8/21). "Tobacco Control Policies: Do They Make a Difference for Low SES Women and Girls?" Journal of Epidemiology and Community Health: Policies that aim to reduce secondhand smoke exposure have been less effective for girls and women of lower economic status than for people of higher economic statues, the report finds. The American Legacy Foundation in the report finds that tobacco control polices have led to a reduction in secondhand smoke exposure for many U.S. residents, and policies such as higher prices have helped to reduce smoking among lower-income women. The report also says women who do not complete high school or who have "blue-collar" jobs are less likely than other women to live in homes without smoke, CQ HealthBeat reports (Abruzzese, CQ HealthBeat, 8/11). "Optimism, Distress, Health-Related Quality of Life, and Change in Cancer Antigen 125 Among Patients With Ovarian Cancer Undergoing Chemotherapy," Psychosomatic Medicine: Janet de Moor of the Ohio State University School of Public Health and colleagues examined the outlook of 90 women with epithelial ovarian cancer at the beginning and end of chemotherapy. De Moor, who conducted the study while at the Dana-Farber Cancer Institute, analyzed the participants' self-reported feelings of optimism and distress, as well as aspects of health-related quality of life among the women, and compared their findings with the women's cancer antigen 125 levels (de Moor et al., Psychosomatic Medicine, 8/1). Decreases in CA 125 levels are used to determine the probability of remission and survival in women with ovarian cancer, Reuters Health reports (Rauscher, Reuters Health, 8/15). This is why this year's World Heart Day taking place on Sunday 28th September is urging people to take action and visit their health-care professional to "Know Your Risk!" "I know my heart is the strongest, most important muscle in my body and a winning performance depends on its vitality," said Stefano Baldini, Olympic marathon gold medal winner. "Having a healthy diet will help keep my heart young and my career long." There are several factors which increase the risk of developing high blood pressure, including the excessive consumption of salt. "However, by adopting lifestyle changes hypertension is controllable," says Professor Shahryar Sheikh, President of the World Heart Federation. Avoiding foods high in salt, refraining from adding salt and being aware of the salt content of food in restaurants and processed foods, could potentially reduce salt consumption by 3 grams a day. Risk factors for heart disease and stroke include high blood pressure, cholesterol and glucose levels, smoking,suspension training straps, inadequate intake of fruit and vegetables, overweight, obesity and physical inactivity. Taken together, these major risk factors account for around 80% of deaths from heart disease and stroke. Out of 23 patients, 16 were treated by vaginal approach and 6 by abdominal approach. Hysterectomy was the major etiology (72.7%) and 9 patients had at least 1 previous abdominal surgery. The vaginal approach allowed early hospital discharge. There were no major intraoperative or early postoperative complications, such as bleeding, blood transfusion or infection. The main reason for the longest hospitalization in those patients with abdominal approach was related to recovering from paralyzed ileum and pain,x factor workout. The students - Suze Gehem, Floor van Goudoever, Albertien Greijdanus and Judith van der Werf - are master students of Industrial Design at the TU Delft. They found this challenging development project for their 'Integral Design Project' thanks to the Vrouw & Verband Foundation (roughly translated: the Women & Bandage/ Connection Foundation). For years now this foundation, currently also involved in this project as subject matter expert, demands attention for and research problems that girls and women experience in regard of menstrual hygiene. According to Dr. Harold Robles, President of the MKI, the current situation around this issue of menstrual hygiene is far from optimal.
