For the study, Tobias Sjoblom of Johns Hopkins University and colleagues compared the DNA codes for 13,023 genes -- about two-thirds of the total number of genes in a cell -- in cells taken from 11 cancerous breast tumors and 11 cancerous colon tumors removed from patients during surgery to DNA codes for noncancerous cells, the Wall Street Journal reports (Regalado, Wall Street Journal, 9/8). According to the study, cancerous tumors contain an average of about 90 "mutant" genes, but only some of those genes contribute to tumor growth. Researchers identified 189 mutant genes that frequently were found in the cancerous tumors (Sjoblom et al., Science, 9/7). Researchers linked 122 of the genes to breast cancer and 69 to colon cancer, and only two genes were linked to both cancers. Although some of the mutated genes might be hereditary, most develop during a person's life, according to Bert Vogelstein, co-director of the Ludwig Center at Johns Hopkins and co-author of the study. "From the first mutation to a full-blown malignancy, it takes literally 20 to 40 years," Vogelstein said, adding, "This data could explain why it takes so long,weider x factor st."     Post-Exercise Caffeine Helps Muscles Refuel

Glycogen, the muscle's primary fuel source during exercise, is replenished more rapidly when athletes ingest both carbohydrate and caffeine following exhaustive exercise, new research from the online edition of the Journal of Applied Physiology shows. Athletes who ingested caffeine with carbohydrate had 66% more glycogen in their muscles four hours after finishing intense, glycogen-depleting exercise, compared to when they consumed carbohydrate alone, according to the study,suspension training straps, published by The American Physiological Society. Caffeine aids carbohydrate uptake It is already established that consuming carbohydrate and caffeine prior to and during exercise improves a variety of athletic performances. This is the first study to show that caffeine combined with carbohydrates following exercise can help refuel the muscle faster. "If you have 66% more fuel for the next day's training or competition, there is absolutely no question you will go farther or faster," said Dr. Hawley, the study's senior author. Caffeine is present in common foods and beverages, including coffee, tea, chocolate and cola drinks. The study was conducted on seven well-trained endurance cyclists who participated in four sessions. The participants first rode a cycle ergometer until exhaustion, and then consumed a low-carbohydrate dinner before going home. This exercise bout was designed to reduce the athletes' muscle glycogen stores prior to the experimental trial the next day. The athletes did not eat again until they returned to the lab the next day for the second session when they again cycled until exhaustion. They then ingested a drink that contained carbohydrate alone or carbohydrate plus caffeine and rested in the laboratory for four hours. During this post-exercise rest time, the researchers took several muscle biopsies and multiple blood samples to measure the amount of glycogen being replenished in the muscle, along with the concentrations of glucose-regulating metabolites and hormones in the blood, including glucose and insulin. The entire two-session process was repeated 7-10 days later. The only difference was that this time, the athletes drank the beverage that they had not consumed in the previous trial. (That is, if they drank the carbohydrate alone in the first trial, they drank the carbohydrate plus caffeine in the second trial, and vice versa.) Neither the researchers nor the cyclists knew which regimen they were receiving, making it a double-blind, placebo-controlled experiment. Glucose and insulin levels higher with caffeine ingestion The researchers found the following: one hour after exercise, muscle glycogen levels had replenished to the same extent whether or not the athlete had the drink containing carbohydrate and caffeine or carbohydrate only four hours after exercise, the drink containing caffeine resulted in 66% higher glycogen levels compared to the carbohydrate-only drink throughout the four-hour recovery period, the caffeinated drink resulted in higher levels of blood glucose and plasma insulin several signaling proteins believed to play a role in glucose transport into the muscle were elevated to a greater extent after the athletes ingested the carbohydrate-plus-caffeine drink, compared to the carbohydrate-only drink Dr. Hawley said it is not yet clear how caffeine aids in facilitating glucose uptake from the blood into the muscles. However, the higher circulating blood glucose and plasma insulin levels were likely to be a factor. In addition, caffeine may increase the activity of several signaling enzymes, including the calcium-dependent protein kinase and protein kinase B (also called Akt), which have roles in muscle glucose uptake during and after exercise. Lower dose is next step In this study, the researchers used a high dose of caffeine to establish that it could help the muscles convert ingested carbohydrates to glycogen more rapidly. However, because caffeine can have potentially negative effects, such as disturbing sleep or causing jitteriness, the next step is to determine whether smaller doses could accomplish the same goal. Hawley pointed out that the responses to caffeine ingestion vary widely between individuals. Indeed, while several of the athletes in the study said they had a difficult time sleeping the night after the trial in which they ingested caffeine (8 mg per kilogram of body weight, the equivalent of drinking 5-6 cups of strong coffee), several others fell asleep during the recovery period and reported no adverse effects.     Gene Variant Linked With Development Of COPD In Men

    Researchers have linked a variant in the vitamin D receptor gene (VDR) with the onset of chronic obstructive pulmonary disease (COPD) in Caucasian men. The study population consisted of participants in the Veterans Administration Normative Aging Study, a multidisciplinary study of aging that began in 1963. The VDR study will be presented at the ATS 2011 International Conference. "Our results show that this gene variant is associated with development of COPD in Caucasian men, and provides support for the notion that vitamin D metabolic pathways may affect COPD risk," said Audrey Poon,rip 60 sale, PhD, postdoctoral fellow at Meakins-Christie Laboratories, McGill University Health Centre in Montreal.     A 51-year-old breast cancer patient from Switzerland has become the first person in the world to be treated using Gated RapidArc, which makes it possible to monitor patient breathing and compensate for tumor motion while quickly delivering radiotherapy during a continuous rotation around the patient. This development from Varian Medical Systems (NYSE: VAR) enables the use of RapidArc radiotherapy technology in combination with respiratory gating a process of turning the beam on and off in synchrony with the patient's breathing. This technique increases the precision of treatment delivery by compensating for tumor motion. In addition, RapidArc enabled the dose to be delivered using fewer than half the dose monitor units needed for an equivalent conventional seven-field IMRT treatment. "We are now using Gated RapidArc to treat all left-sided breast cancer patients when there is a need to include the supraclavicolar and/or internal mammary chain lymph nodes, or when chest wall anatomy will affect the dose to organs we want to protect," said Dr. Nicolini. "We are also planning to use the technique for lung and other thoracic and abdominal tumors when suitable." Women doctors are more likely than their male counterparts to believe that women are specifically susceptible to depression during two key times of hormonal transition in a woman's life: puberty (67.5 percent to 48.2 percent) and perimonpause (92.8 percent to 67.5 percent). There was no difference in the beliefs of male and female doctors about the risk of depression in the postpartum or post menopause periods. When asked what the major symptoms of depression are, female doctors were significantly more likely to mention lack of motivation or giving up easily and being unable to concentrate. Trouble sleeping, lack of energy or fatigue, not feeling well, feeling sad, crying, changes in appetite, and a loss of interest in friends, activities or social life were the leading responses for both sexes. "The beliefs of female and male doctors about women and depression are pretty consistent," said Jo Parrish, vice president of communications for the Society, "but our survey does reveal a few areas where female doctors may be more in tune with the changes women undergo and the feelings they experience, which is not unexpected,trx bands for sale.     Genetic Link Between Two Rare Diseases Established By Next G

    4 (1 votes) Scientists have successfully used "next generation sequencing" to identify mutations that may cause a rare and mysterious genetic disorder. Some individuals with Perrault syndrome also have neurological symptoms. Prior to the current study, no genes for Perrault syndrome had been identified. A research group led by Mary-Claire King, PhD, from the University of Washington in Seattle studied the genetics of Perrault syndrome in a small family, originally of Irish and Italian ancestry, that included two sisters with well-characterized Perrault syndrome. "Because the family is small and not consanguineous (both parents descended from a common ancestor), standard genetic mapping techniques would not have been informative in identifying the responsible gene," explains Dr. King. "Instead, we attempted to identify the gene responsible for Perrault syndrome in this family through the use of whole exome sequencing." The exome can be thought of as a kind of genetic blueprint for the synthesis of proteins. After sequencing the entire exome of one of the sisters, the researchers identified a single gene (HSD17B4) that exhibited two rare functional variants. This gene encodes D-bifunctional protein (DBP), a multifunctional enzyme involved in lipid metabolism. Underscoring the genetic diversity of the disease, the researchers went on to show that six other families with Perrault syndrome had normal HSD17B4. "Other mutations in HSD17B4 are known to cause a very severe congenital syndrome called DBP deficiency that is generally fatal within the first two years of life,trx force kit tactical," says Dr. King. "No girls with DBP deficiency have been reported to survive past puberty, so ovarian abnormalities have not previously been known to be associated with this illness. The few reported long term survivors of DBP deficiency exhibit hearing loss and neurological dysfunction." Women with a harmful mutation in the BRCA1 or BRCA2 gene have a high lifetime risk of ovarian cancer (range, 15 percent-54 percent), and mutations in either of these genes increase susceptibility to cancers of the ovary, fallopian tube, and peritoneum (abdominal lining), according to background information in the article. Women with this mutation are often advised to undergo preventive oophorectomy (surgical removal of the ovaries). The effectiveness of this intervention has not been prospectively evaluated in a large group of patients. Steven A. Narod, M.D., of the Toronto-Sunnybrook Regional Cancer Center, Toronto, Ontario, and colleagues conducted a study to determine the absolute risks for developing ovarian, fallopian tube, and peritoneal cancers in a group of BRCA1 and BRCA2 mutation carriers and estimated the risk reduction associated with salpingo-oophorectomy (surgical removal of the ovaries and fallopian tubes). Women known to carry a BRCA1 or BRCA2 mutation were identified from an international registry between 1992 and 2003. A total of 1,828 carriers at 1 of 32 centers in Canada,discount trx suspension trainer, the United States, Europe, and Israel completed questionnaires at baseline and follow-up. Participants were observed from the date of study entry until: diagnosis of ovarian, fallopian tube, or peritoneal cancer; death; or the date of the most recent follow-up. After an average follow-up of 3.5 years, 50 new ovarian, fallopian tube, and peritoneal cancer cases were reported in the group. Of the 1,828 women, 555 (30 percent) underwent preventive removal of the ovaries and tubes prior to study entry, 490 (27 percent) underwent the procedure after entering the study, and 783 (43 percent) did not undergo the procedure. There were 32 new cancers diagnosed in women with intact ovaries,trx training bands. The estimated cumulative incidence of peritoneal cancer is 4.3 percent at 20 years after oophorectomy. The overall (adjusted) reduction in cancer risk associated with bilateral oophorectomy is 80 percent. The researchers estimated the risk of ovarian cancer to be 62 percent for BRCA1 carriers and 18 percent for BRCA2 carriers in women up to age 75 with both ovaries intact. "Women who carry a mutation in the BRCA1 gene are asked to consider prophylactic bilateral salpingo-oophorectomy at age 35 or thereabouts,weider x factor home gym, in order to reduce the risk of ovarian, fallopian tube, and breast cancer. Our observations support this recommendation. It may be reasonable to wait until a time closer to menopause to prevent ovarian and fallopian tube cancer in BRCA2 carriers but this delay will diminish the level of protection offered against breast cancer in this subgroup," the authors write. "We estimate the magnitude of the risk reduction [from preventive removal of the ovaries and tubes] to be approximately 80 percent and the residual risk of 4 percent of peritoneal cancer is not sufficiently high to recommend against the procedure. It is important that both the fallopian tubes and ovaries be removed because either site may be the origin of cancer and both organs should be examined in fine detail to rule out the presence of microscopic disease," the researchers conclude.     Falls More Likely In Elderly Men With Low Testosterone Level

    Low testosterone levels may be associated with a higher risk of falling in older men, according to a report in the October 23 issue of Archives of Internal Medicine, one of the JAMA/Archives journals. The level of testosterone in a man's blood--including the amount that is bioavailable,aerosling suspenstion trainer, or able to be used by the body--declines as he ages,trx at home, according to background information in the article. Some researchers have speculated that this decline in sex hormone levels contributes to health conditions associated with aging; however, few studies support this notion. Muscle weakness and muscle loss are two adverse events that often occur with age and are known to increase the risk of falling among older adults. Eric Orwoll, M.D,trx shop., Oregon Health & Science University, Portland,trx force training kit, and colleagues studied 2,587 men age 65 to 99 (average age 73) who enrolled in the Osteoporotic Fractures in Men (MrOS) Study between 2000 and 2002. At the beginning of the study, blood samples were taken and participants filled out questionnaires about their medical history, medications and lifestyle habits. The men completed several physical performance tests, including those assessing grip strength, leg power and balance. Every four months, they reported whether they had fallen and if so, how many times. During the course of the study, which continued until March 2005, 56 percent of the men fell at least once. Those with lower bioavailable testosterone levels were significantly more likely to fall and to fall multiple times than those with higher levels--among the one-fourth of participants with the lowest testosterone levels, the risk for falling was 40 percent higher than among the one-fourth with the highest testosterone levels. The association was stronger in younger men (ages 65 to 69) and not apparent in men older than 80. The association remained the same when the researchers factored in the scores on physical performance tests. "Bioavailable testosterone concentration is associated with measures of physical performance, but the association of testosterone level to the risk of falling is apparent regardless of physical performance," the authors write. "Thus, the mechanisms by which testosterone level affects the propensity to fall may involve other pathways." For example, low testosterone levels could impair vision, thinking processes or coordination, increasing the risk for falls. "These findings strengthen the link between testosterone and the health of older men, suggesting that the effects of testosterone on fall risk may be via novel mechanisms and provide insight into how testosterone measurements might be useful for identifying men at higher risk for adverse events," the authors conclude. "Moreover, these results provide additional justification for trials of testosterone supplementation in older men and should aid in the design of those studies." ### (Arch Intern Med. 2006,trx pro;166:2124-2131.)     Low-fat Diet's Benefits For Women Less Than Expected, Stanfo

    Older women who simply reduce the amount of total fat in their diet won't generally lower their risk of breast or colorectal cancer or heart disease, according to the latest findings from the federally funded Women's Health Initiative. But a Stanford University School of Medicine researcher who helped direct the WHI work said the study showed a modest reduction in breast cancer among the women who started with the highest fat intake before cutting back. And it also suggested a health benefit for women who reduced their consumption of saturated and trans fats. "Just switching to low-fat foods is not likely to yield much health benefit in most women," said Marcia Stefanick, PhD, professor of medicine at the Stanford Prevention Research Center and chair of the WHI steering committee. "Rather than trying to eat 'low-fat,' women should focus on reducing saturated fats and trans fats." She also recommended that women eat more vegetables, in particular dark,trx suspension trainer sale, leafy greens and cruciferous vegetables, though the trial did not specifically study these foods. The dietary findings are reported in three studies to be published in the Feb. 8 issue of the Journal of the American Medical Association. The studies showed that women on the low-fat diet experienced a 9 percent reduction in the incidence of breast cancer - but this was not statistically significant,x factor workout machine. (For instance, in a group of 10,000 women, 42 in the low-fat diet group developed breast cancer each year compared with 45 in the group maintaining their regular eating patterns.) The dietary changes made no appreciable difference in the rates for colorectal cancer and cardiovascular disease. The findings are the latest clinical results from the WHI, a 15-year, broad-based look at the causes and prevention of diseases affecting older women. Previous WHI studies have involved hormone therapy,trx suspension trainer pro pack, heart disease and osteoporosis. The low-fat diet study involved nearly 49,000 postmenopausal women between the ages of 50 and 79 who were tracked over the course of a little more than eight years on average, making it the nation's largest long-term study of a low-fat diet. The goal was to test a widely held theory that low-fat diets helped reduce the risk of cancer and heart disease. Forty percent of the participants were assigned to the low-fat diet, in which they were asked to reduce their fat intake to 20 percent of their total calories while eating five or more daily servings of vegetables and fruits and six servings of grains,trx suspension trainer pro. The remaining 60 percent served as a comparison group and did not change their diet. Although the primary hypotheses of protective effects of a low-fat diet on breast and colorectal cancer failed the test, the WHI researchers pointed out that the majority of women assigned to the low-fat diet didn't meet the 20 percent fat goal: On average, the women reduced their fat intake to 24 percent in the first year, but slowly increased their fat intake to 29 percent by the eighth year. Furthermore, the study showed that women who had the highest fat intake at the study's outset showed greater evidence for reducing their breast cancer risk on the diet program. There was also a suggestive trend of breast cancer risk reduction for women who initially had the lowest consumption of vegetables and fruits and then increased their intake by one serving per day as part of the diet. Regarding the heart disease findings, Stefanick noted that the women weren't asked to differentiate between "good fats" (the unsaturated fats found in fish, nuts and vegetable oils) and "bad fats" (saturated fats and trans fats found in processed foods, meats and some dairy products), which is emphasized in current guidelines for heart disease reduction. "This shows that you can't rely on using low-fat substitutes to make a difference,exercise trx," Stefanick said. "You really need to think about what kinds of fats you're eating and the foods that should be part of your diet, such as vegetables, for instance." For women who want to maintain their health, Stefanick advised them to follow a diet low in saturated and trans fats, and rich in vegetables and fiber - rather than to strive to eat "low-fat" foods. She also advised them to pay attention to total caloric intake regardless of diet composition and to get adequate regular exercise. Women should also get routine mammograms and screenings for colorectal cancer and heart disease risk, including checking their cholesterol profile, blood pressure, blood sugar and body weight. The low-fat diet study is one of three randomized clinical trials that are part of the WHI. A previous trial on hormone therapy showed that estrogen and a combination of estrogen and progestin increased risks in such diseases as stroke, blood clots and breast cancer among postmenopausal women.
